By enrolling in the New York State Donate Life Registry, you are giving legal consent for the recovery of your organs, tissues and eyes for the purposes of transplantation and research at the time of your death. Donor designation in the registry:
Legally empowers you to save the lives of others.
Grants you the right to self-determination and self-choice. It allows you to take control of your life and the end-of-life decision-making process. No one, including your next of kin, can reverse your decision to save lives when you add your name to the database.
Registry information is kept strictly confidential and can only be accessed by:
Department of Heath employees when required for the performance of their official duties
Federally regulated organ procurement organizations such as the New York Organ Donor Network
New York State licensed tissue and eye banks
Other entities formally approved by the Commissioner. Such access can only be for the purpose of identifying potential donors at or near the time of death.
